Purpose: To create a registration form with clear program options
and instructions for a smoother registration process
Scope of Project
For this project I first had to analyze the program options, understand the customer needs
and then create the layout that would allow for a smooth and efficient registration process.
The main challenge was trying to incorporate the different service options on one page
to minimize printing costs and to streamline their filing system. In order to accomplish this, I
created a double sized page with the single program options on on side and the
membership options on the other side.
